Understanding Tourniquets and When They Are Needed
A tourniquet is a medical device designed to stop or reduce blood flow to a limb during a severe bleeding emergency. The word comes from French and literally means "to turn" — because traditional tourniquets involved twisting a piece of material to tighten it around an arm or leg. Modern tourniquets work on the same principle but with improved designs that make them safer and more reliable.

Tourniquets are used specifically for life-threatening bleeding that cannot be controlled through other methods like direct pressure or elevation. According to the American College of Surgeons, severe extremity hemorrhage — bleeding from an arm or leg — accounts for a significant portion of preventable deaths from trauma. Research shows that tourniquets have become standard equipment in emergency medicine, military field medicine, and public access locations like schools and workplaces.
The main situations where a tourniquet becomes necessary include:
- Severe lacerations or cuts that sever major blood vessels in an arm or leg
- Traumatic amputation where part or all of a limb is severed
- Crush injuries that damage blood vessels and cannot be managed with pressure alone
- Multiple serious wounds on the same limb that make direct pressure impractical
- Situations where the person applying first aid needs both hands free to help the injured person or manage other life threats
It is important to understand that tourniquets are not meant for minor cuts or bleeding that responds to direct pressure. Using a tourniquet inappropriately can cause tissue damage. Medical professionals distinguish between situations requiring a tourniquet and situations where other bleeding control methods are appropriate. Learning to recognize the difference is a core part of understanding emergency response.
Practical takeaway: Tourniquets address life-threatening limb bleeding when direct pressure and other methods do not work. Understanding what constitutes severe bleeding helps determine when a tourniquet is the right choice.
Types of Tourniquets and How They Work
Several types of tourniquets exist, and they operate using different mechanisms. The two main categories are pneumatic tourniquets and mechanical tourniquets. Pneumatic tourniquets use air pressure to compress an entire limb and are primarily used in surgical settings by trained medical professionals. Mechanical tourniquets, which are the focus of most first aid and emergency response training, use physical tightening mechanisms and are designed for field use.

Modern commercial mechanical tourniquets come in several designs. The Combat Application Tourniquet (CAT), developed by military medics and now widely used in civilian emergency settings, features a windlass mechanism — a rod that twists to tighten a strap around the limb. The strap is typically made of durable material like nylon, and the windlass system provides mechanical advantage, meaning the person applying it does not need to be extremely strong to achieve adequate tightness.
Another common design is the tourniquet with a ratcheting buckle system. These use a strap that threads through a buckle mechanism, similar to a seat belt, and can be tightened by pulling the loose end. Some versions include a secondary locking mechanism to prevent accidental loosening.
The basic principle behind all mechanical tourniquets is straightforward: the device compresses the limb tightly enough to stop blood flow to areas below the tourniquet. This requires pressure that exceeds the systolic blood pressure — the highest pressure in arteries during heartbeats. Most tourniquets are designed to compress limbs to pressures between 150 and 200 millimeters of mercury, which is higher than typical resting blood pressure and sufficient to stop arterial bleeding.
Tourniquets are typically placed on the upper arm between the elbow and shoulder, or on the upper leg between the hip and knee. These locations work better than lower limbs because they have fewer arteries and nerves to compress. Placement above a wound is essential — the tourniquet must be positioned between the wound and the heart to be effective.
Practical takeaway: Understanding tourniquet mechanics helps explain why placement, tightness, and technique matter. Different designs accomplish the same goal through slightly different methods, but all depend on applying sufficient pressure to stop arterial bleeding.
Step-by-Step Application Techniques
Applying a tourniquet correctly is important for its effectiveness and to minimize the risk of tissue damage. While specific techniques vary slightly depending on the tourniquet model, the general process follows established guidelines taught by organizations like the American Red Cross and military combat medic training programs.

The first step is to recognize that a tourniquet is needed. The person should be assessed for severe, uncontrolled bleeding in an extremity. If blood is pumping out rapidly or does not slow with direct pressure applied for at least several minutes, a tourniquet may be indicated. The person applying the tourniquet should wear gloves if available to reduce infection risk.
Next, the tourniquet should be positioned on the limb. For arm bleeding, it goes on the upper arm, typically 2 to 3 inches below the armpit. For leg bleeding, it goes on the upper thigh, several inches below the groin. The tourniquet should be placed directly against the skin or over a single layer of thin clothing, not over heavy jackets or multiple layers that could cause uneven compression. The limb should be exposed enough to allow the rescuer to see the tourniquet and verify its placement.
The tourniquet is then tightened using the appropriate mechanism for that model. If using a windlass-style tourniquet, the strap is wrapped around the limb and secured, then the rod is twisted until the tourniquet is snug and does not slip. The person should continue tightening until bleeding from the wound stops. This typically requires more tightness than might initially seem necessary. Some training emphasizes tightening until the injured person reports the limb feels numb or until resistance increases significantly during tightening.
After tightening, the tourniquet must be locked or secured so it cannot loosen. Most commercial tourniquets have a locking mechanism for this purpose. The time the tourniquet was applied should be noted, either by writing the time on the tourniquet itself with a marker, on a piece of tape attached to the tourniquet, or on the person's skin. This time notation is critical for hospital staff to know how long the limb has been without blood flow.
Throughout the process, the tourniquet should be checked to ensure it stays tight. The person should look for signs of bleeding below the tourniquet; if blood is still flowing from the wound, the tourniquet may not be tight enough. If the tourniquet slips or appears to loosen, it should be tightened again.
Practical takeaway: Correct tourniquet application involves placement above the wound, proper tightening until bleeding stops, securing it against loosening, and noting the application time. Following these steps increases the likelihood of stopping severe bleeding and helps medical professionals provide appropriate care.
What to Do After Applying a Tourniquet
Applying a tourniquet is not the end of emergency care; it is one step in a larger response. After a tourniquet is applied, the injured person needs medical attention from trained professionals, typically through emergency medical services or hospital care. The person applying the tourniquet should continue monitoring the injured person's overall condition and be prepared to describe what happened to emergency responders.

Medical professionals will eventually assess whether the tourniquet should stay in place, be released, or be replaced with another method of bleeding control. This decision depends on the nature of the injury, how long the tourniquet has been in place, whether surgical intervention is needed, and other factors that trained medical staff can evaluate. In a hospital setting, doctors may release the tourniquet gradually while monitoring for bleeding, or they may proceed directly to surgery for severe injuries.
It is important to recognize that leaving a tourniquet in place for extended periods can cause tissue damage. Muscle and nerve tissue can begin to deteriorate after several hours without blood flow. However, research suggests that tissue damage is unlikely within the first 6 hours of tourniquet application, and even beyond that timeframe, a tourniquet is still preferable to death from uncontrolled bleeding. Modern guidelines prioritize saving life over preserving a limb in extreme situations.
If a tourniquet is applied in a remote location where medical care will be delayed, the situation becomes more complex. In wilderness rescue or prolonged
