Understanding Asthma and How Medications Work
Asthma is a chronic lung condition that affects millions of people worldwide. According to the CDC, about 8.4% of American adults and 7% of children have asthma. When a person has asthma, the airways in their lungs become inflamed and narrow, making it difficult to breathe. This can happen suddenly during an asthma attack, or develop gradually over time.

The airways of people with asthma are sensitive to certain triggers like allergens, cold air, exercise, or infections. When exposed to these triggers, the muscles around the airways tighten, the lining of the airways swells, and the body produces extra mucus. All three of these changes make the airways narrower, which restricts airflow. This causes symptoms like wheezing, coughing, chest tightness, and shortness of breath.
Asthma medications work by addressing these three problems in different ways. Some medications reduce inflammation in the airways, while others relax the tight muscles around the airways. Understanding how these medications function helps people with asthma make informed decisions about their treatment plans with their healthcare provider.
The severity of asthma varies greatly from person to person. Some people experience only occasional symptoms during allergy season or when exercising. Others deal with daily symptoms that interfere with work, school, or sleep. The type and amount of medication needed depends on how often symptoms occur and how severe they are. A healthcare provider can assess an individual's asthma to determine the right treatment approach.
Practical Takeaway: Learning the basic mechanics of how asthma affects the lungs and how medications counter these effects can help you have better conversations with your healthcare provider about your symptoms and treatment options.
Controller Medications for Long-Term Asthma Management
Controller medications are taken regularly every day to prevent asthma symptoms from occurring in the first place. These medications work by reducing inflammation in the airways and keeping them open. People with persistent asthma—even those with mild symptoms that happen several times a week—typically benefit from controller medications. Think of controller medications as preventive medicine that keeps asthma under control over weeks and months.

Inhaled corticosteroids are the most commonly prescribed controller medications. Examples include fluticasone, budesonide, and mometasone. These medications reduce inflammation and mucus production in the airways. Corticosteroids used in inhalers deliver medication directly to the lungs at lower doses than oral corticosteroids, which means fewer side effects. Studies show that inhaled corticosteroids are very effective at reducing asthma symptoms and preventing attacks when used consistently.
Long-acting beta-2 agonists (LABAs) are another type of controller medication. These medications relax the muscles around the airways, keeping them open for 12 to 24 hours. Common LABAs include salmeterol and formoterol. These medications are typically combined with inhaled corticosteroids in one inhaler rather than used alone. This combination therapy is more effective than either medication used separately.
Other controller medications include leukotriene modifiers, which reduce inflammation caused by certain immune system chemicals, and long-acting muscarinic antagonists (LAMAs), which keep airways open by blocking certain nerve signals. Some people take combination inhalers that contain multiple types of controller medications in one device. The specific controller medication prescribed depends on the person's asthma severity, age, and response to treatment.
Controller medications must be taken regularly, even when someone feels fine and has no symptoms. This is crucial because inflammation in the airways may be present even without noticeable symptoms. Missing doses or stopping controller medications can lead to increased inflammation and more frequent or severe attacks. Most people taking controller medications start seeing improvement within days or weeks, though maximum benefit may take several weeks.
Practical Takeaway: Controller medications are maintenance treatments taken daily to prevent symptoms. Understanding that these medications work best when used consistently, even during symptom-free periods, helps people achieve better long-term asthma control.
Rescue Medications for Acute Asthma Symptoms
Rescue medications, also called reliever or quick-relief medications, work quickly to open airways during an asthma attack or when symptoms suddenly appear. The most common rescue medication is the short-acting beta-2 agonist (SABA) inhaler, with albuterol being the most frequently used. When someone experiences sudden wheezing, coughing, chest tightness, or shortness of breath, a few puffs from a rescue inhaler can bring relief within minutes.

Albuterol and other SABAs work by relaxing the muscles around the airways almost immediately. Most people feel relief within 15 minutes of using a SABA inhaler. The effects typically last four to six hours. Unlike controller medications, rescue inhalers do not reduce inflammation—they simply relieve the symptoms of a current attack by opening narrowed airways. This is why rescue inhalers are sometimes called "blue inhalers" because they often come in blue canisters.
How frequently someone needs to use a rescue inhaler provides important information about asthma control. If someone uses a rescue inhaler more than twice a week (not counting use before exercise), this suggests their asthma is not well controlled with their current controller medication. People should discuss increased rescue inhaler use with their healthcare provider, as it may indicate a need to adjust their treatment plan.
Rescue inhalers are portable and quick, making them useful for unexpected symptoms or for preventing exercise-induced asthma. Some people take one or two puffs of their rescue inhaler before exercise to prevent symptoms during physical activity. During a severe asthma attack where a rescue inhaler does not provide relief, or if someone has difficulty breathing or chest pain, emergency medical care is necessary. People with asthma should always have their rescue inhaler with them.
It is important to distinguish between rescue medications and controller medications. A rescue inhaler is not a substitute for a controller medication. People with persistent asthma need both—a daily controller medication to prevent symptoms and a rescue inhaler for when symptoms occur despite prevention. Relying only on a rescue inhaler without taking a controller medication means asthma is likely not well managed.
Practical Takeaway: Rescue inhalers provide fast symptom relief but do not address the underlying inflammation causing asthma. Tracking how often you use your rescue inhaler helps identify whether your current asthma control plan needs adjustment.
Biologic Medications for Severe Asthma
Biologic medications represent a newer approach to treating severe asthma that does not respond well to standard controller medications. These drugs are created from living cells and work by targeting specific parts of the immune system involved in asthma inflammation. Several biologic medications are now available, each designed for particular types of severe asthma.

Monoclonal antibody medications like omalizumab, dupilumab, and reslizumab target different immune system molecules that trigger asthma inflammation. Omalizumab targets immunoglobulin E (IgE), which is involved in allergic asthma. This medication may benefit people whose asthma is triggered by allergies. Dupilumab targets interleukin-4 receptors and works for both allergic and eosinophilic asthma. Reslizumab targets interleukin-5 and is used for eosinophilic asthma, which involves high levels of a certain type of white blood cell called eosinophils.
The National Institutes of Health reports that biologic medications can significantly reduce asthma attacks and hospitalizations in people with severe asthma. Some studies show that these medications reduce exacerbations by 50% or more in people with appropriate types of severe asthma. However, biologic medications are expensive and are typically prescribed only after other treatment options have been tried or when asthma is particularly severe.
Biologic medications are given either by injection or infusion. Some are given monthly, while others are given every few weeks. People taking biologic medications require ongoing monitoring by their healthcare provider to assess effectiveness and watch for any side effects. Blood tests may be needed before starting biologic therapy to determine which medication might work best based on immune system markers.
Not everyone with severe asthma benefits from biologic medications. These medications work best in specific types of severe asthma defined by particular immune system characteristics. A healthcare provider can order tests to
