Understanding Diarrhea: Causes and When to Seek Medical Care
Diarrhea happens when your digestive system moves food through your intestines too quickly, or when your intestines absorb less water than normal. This results in loose or watery stools that occur more frequently than your usual bathroom habits. Most people experience diarrhea at least once or twice per year, making it one of the most common digestive complaints.

Several factors can trigger diarrhea. Viral infections—particularly from norovirus and rotavirus—cause acute diarrhea that typically lasts a few days. Bacterial infections from contaminated food or water, such as those caused by Salmonella or E. coli, can produce more severe symptoms. Parasitic infections, though less common in developed countries, also cause diarrhea. Beyond infections, dietary triggers include consuming high-fat foods, foods with artificial sweeteners, dairy products (if you're lactose intolerant), or foods containing sorbitol. Certain medications, especially antibiotics, can disrupt your gut bacteria and cause diarrhea as a side effect.
Stress and anxiety can physically affect your digestive system, leading to diarrhea. Inflammatory bowel conditions like Crohn's disease or ulcerative colitis cause chronic diarrhea. Travel-related diarrhea occurs when you encounter unfamiliar bacteria in different geographic locations. Food allergies and celiac disease also produce diarrheal symptoms when you consume trigger foods.
Knowing when diarrhea requires medical attention matters for your health. Contact a doctor if diarrhea lasts longer than two days in adults, if you experience severe abdominal pain or cramping, if you notice blood or black stools, if you develop signs of dehydration (extreme thirst, dry mouth, little or no urination, severe weakness), or if you have a fever above 102°F. Children and elderly adults need medical evaluation sooner, typically within 24 hours of symptom onset. People with weakened immune systems should also seek medical care promptly.
Practical takeaway: Keep a simple symptom log noting when diarrhea started, what you ate beforehand, and any other symptoms. This information helps you identify patterns and provides valuable details for your doctor if medical care becomes necessary.
The Role of Hydration in Diarrhea Management
Dehydration represents the primary health risk during diarrhea. When you have diarrhea, your body loses fluids and electrolytes—minerals like sodium, potassium, and chloride that regulate fluid balance, muscle function, and nerve signaling. According to the Centers for Disease Control and Prevention, severe dehydration from diarrhea causes approximately 2,195 deaths annually in the United States, though most cases respond well to proper fluid replacement.

Water alone doesn't fully replace what your body loses during diarrhea. While drinking water prevents dehydration, oral rehydration solutions (ORS) work more effectively because they contain the right balance of water, salts, and sugar. The World Health Organization recommends ORS as the gold standard treatment for diarrhea-related dehydration. Common ORS products include Pedialyte, Liquid IV, and DripDrop. Store-brand versions work equally well and cost less. You can also make a simple ORS at home by mixing one liter of clean water with six teaspoons of sugar and one-half teaspoon of salt.
Drink small amounts frequently rather than large quantities at once. Sipping six to eight ounces every 15 to 20 minutes works better than drinking a full glass at one time, which may cause more cramping and additional loose stools. If you experience nausea, try drinking through a straw or using a spoon to take small amounts. Let carbonated ORS (if using that type) go flat before drinking. Continue drinking rehydration solutions for several hours after your last loose stool, even if you feel better.
Certain beverages should be avoided during diarrhea. Avoid alcohol, caffeine, and drinks high in sugar (like soda and juice), as these can worsen diarrhea. Milk and dairy products may intensify symptoms if you're temporarily lactose intolerant during illness. Sports drinks designed for athletic performance contain too much sugar and not enough electrolytes for diarrhea treatment.
Practical takeaway: Purchase an ORS product or the ingredients to make homemade rehydration solution before you need it. Keep it stocked in your medicine cabinet so you're prepared when diarrhea strikes.
Dietary Approaches During and After Diarrhea
The outdated advice to avoid eating during diarrhea has been replaced by current medical guidance supporting early, gentle nutrition. Eating appropriate foods helps your body maintain strength and supports recovery. The BRAT diet—bananas, rice, applesauce, and toast—represents a starting point for bland foods that are easier to digest.

Bananas provide potassium, which your body loses during diarrhea. They're soft and easy to digest. White rice offers simple carbohydrates without added fiber that might irritate your digestive system. Applesauce (unsweetened) provides additional potassium and pectin, a soluble fiber that may help firm stools. Plain toast without butter or toppings provides carbohydrates. Beyond the BRAT foods, consider adding saltine crackers, plain chicken or turkey, boiled eggs, white bread, and low-fat yogurt containing live cultures (which may help restore beneficial gut bacteria).
Certain foods should be avoided while diarrhea is active. High-fat foods like fried items, butter, and fatty meats slow digestion and can worsen symptoms. Spicy foods irritate the digestive tract. High-fiber foods like whole grains, beans, raw vegetables, and fruits with skin increase intestinal movement when your system is already working overtime. Lactose-containing dairy products may cause problems. Artificial sweeteners and sugar alcohols (found in sugar-free gum and candies) can trigger additional loose stools.
Timing matters during recovery. Start eating when you feel ready, usually within a few hours of your last loose stool. Begin with small amounts—perhaps half a cup of applesauce or a few saltine crackers. Wait 30 minutes to see if your stomach tolerates it, then gradually increase portion sizes. After diarrhea resolves completely, spend another 24 to 48 hours eating bland foods before returning to your normal diet. This gradual transition prevents triggering another episode.
Practical takeaway: Stock your kitchen with non-perishable BRAT diet items: white rice, unsweetened applesauce, crackers, and shelf-stable toast. These items have long shelf lives and cost little, making them convenient to have on hand.
Natural and Over-the-Counter Remedies for Diarrhea Relief
Several over-the-counter medications can reduce diarrhea symptoms. Bismuth subsalicylate (Pepto-Bismol) reduces inflammation in your digestive tract and decreases the number of loose stools. Typical dosing involves taking one dose every 30 to 60 minutes, up to eight doses per day. However, bismuth subsalicylate shouldn't be used if you have a fever, as it may mask serious infections. It can also turn your tongue and stools black, which is harmless but startling.

Loperamide (Imodium) works by slowing intestinal movement, reducing the frequency of loose stools. It works quickly and typically shows effects within 30 minutes to an hour. However, loperamide shouldn't be used if you have bloody stools or high fever, as slowing your intestines may trap harmful bacteria and worsen infection. Diphenoxylate with atropine (Lomotil) functions similarly but requires a prescription.
Beyond pharmaceutical options, several natural remedies show promise based on traditional use and some scientific evidence. Ginger root has anti-inflammatory properties and may settle your stomach. Drink ginger tea by steeping fresh ginger slices in hot water. Peppermint tea has been used for centuries to ease digestive discomfort, though evidence is limited. Chamomile tea may help reduce intestinal cramping. Probiotics—live beneficial bacteria—may help restore your gut microbiome, particularly after antibiotic use. Yog
