Understanding Common Causes of Shoulder Pain in Older Adults
Shoulder pain affects a significant portion of the senior population. Research shows that approximately 16-26% of adults over 65 experience shoulder pain at some point. The shoulder is a complex joint with multiple bones, muscles, tendons, and ligaments working together, which means there are many potential sources of discomfort.

One of the most frequent causes is rotator cuff disease, which involves the group of muscles and tendons surrounding the shoulder joint. As people age, these tendons naturally wear down and become more prone to tears. Studies indicate that rotator cuff tears occur in about 20-30% of people over 60, though not all tears cause pain or limit function.
Arthritis is another common condition affecting seniors' shoulders. Osteoarthritis develops when the protective cartilage in the shoulder joint gradually wears away over time. This typically develops more slowly than rotator cuff problems and is often associated with stiffness and aching rather than sharp pain.
Other conditions that frequently cause shoulder pain in older adults include:
- Frozen shoulder (adhesive capsulitis), which causes stiffness and limits movement
- Bursitis, inflammation of the small fluid-filled sacs that cushion the shoulder joint
- Tendinitis, inflammation of the tendons connecting muscles to bone
- Bone spurs or calcification within the shoulder joint
- Referred pain from the neck or upper back
Many seniors develop shoulder pain following an injury, such as a fall or sudden movement. Others experience gradual onset without any specific incident. Age-related changes in the shoulder structures, combined with years of use and possible repetitive activities, create an environment where pain can develop.
Practical Takeaway: Understanding what might be causing shoulder pain is the first step toward exploring treatment options. Different conditions respond to different approaches, so identifying the likely cause helps narrow down which treatment strategies may work best.
Physical Therapy and Exercise-Based Treatments
Physical therapy represents one of the most researched and commonly recommended approaches for shoulder pain in seniors. Multiple studies demonstrate that structured exercise programs can reduce pain and improve shoulder function in older adults. A physical therapist can design a program tailored to an individual's specific condition, age, and fitness level.

Physical therapy typically involves several components. Stretching exercises help maintain and improve the range of motion in the shoulder. Strengthening exercises target the rotator cuff muscles and the larger shoulder stabilizer muscles. These exercises are typically performed with low resistance and high repetition, making them suitable for seniors. Therapists also teach proper posture and body mechanics to reduce strain on the shoulder during daily activities.
The evidence supporting physical therapy is substantial. Research published in medical journals shows that people with rotator cuff problems who participated in 6-12 weeks of physical therapy experienced significant pain reduction. For frozen shoulder, structured therapy combined with gentle stretching can help restore movement over time, though improvement typically takes several months.
Physical therapy sessions generally occur 2-3 times per week for 4-12 weeks, depending on the condition and severity. Between sessions, therapists provide home exercise programs that patients perform daily. Consistency is important—studies show that people who complete their home exercises see better outcomes than those who only attend sessions.
Common exercises used in shoulder therapy include:
- Pendulum exercises, where the arm hangs and swings gently in circles
- Wall walks, gradually moving fingers up a wall to increase range of motion
- Resistance band exercises for rotator cuff strengthening
- Scapular stabilization exercises to support proper shoulder blade movement
- Cross-body stretches to address tightness
Physical therapy can be delivered in outpatient clinics, hospitals, or through home visits. Some insurance plans cover physical therapy for seniors, though coverage varies. The cost typically ranges from $50-$150 per session depending on location and facility type.
Practical Takeaway: Physical therapy offers a structured, evidence-based approach that addresses both pain and function. Starting therapy early and committing to the home exercise program significantly improves the likelihood of successful outcomes.
Medical and Pharmaceutical Treatment Options
Several pharmaceutical and medical interventions can help manage shoulder pain in seniors. These approaches work through different mechanisms and are often used alongside other treatments like physical therapy.

Over-the-counter pain relievers are frequently the first step in managing shoulder pain. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen reduce both pain and inflammation. However, seniors need to use NSAIDs carefully, as prolonged use can cause stomach issues, kidney problems, or increase cardiovascular risk—especially in those with existing health conditions. Acetaminophen is an alternative that doesn't carry the same risks but doesn't reduce inflammation.
Prescription medications may be considered for more severe pain. These might include muscle relaxants, which can reduce muscle tension around the shoulder, or stronger pain medications for short-term use. Doctors typically recommend using the lowest effective dose for the shortest period necessary.
Corticosteroid injections directly into the shoulder joint or surrounding structures can provide significant pain relief, particularly for bursitis, tendinitis, and arthritis. Studies show that corticosteroid injections often provide relief within days that can last weeks to months. A typical course might involve 1-3 injections spaced several weeks apart. However, repeated injections over time may weaken tissues, so doctors generally limit the number of injections a person receives.
Hyaluronic acid injections, sometimes called viscosupplementation, are another option for shoulder arthritis. This substance mimics the fluid naturally present in joints and may reduce pain by improving joint lubrication. These injections typically require a series of 3-5 injections given weekly.
Platelet-rich plasma (PRP) therapy and stem cell treatments are newer options that some clinics offer for shoulder pain. These treatments involve injecting concentrated components from the patient's own blood or stem cells into the damaged area. While research is ongoing, current evidence suggests these may help with certain conditions, though more studies are needed. These treatments are typically not covered by insurance and can cost $500-$2,000 per injection.
Practical Takeaway: Medications and injections can provide pain relief that makes it possible to participate in physical therapy and other active treatments. These approaches work best when combined with exercise and activity modification rather than used alone.
Non-Invasive Therapies and Pain Management Techniques
Beyond physical therapy and medication, several non-invasive approaches can help manage shoulder pain. These techniques often have minimal side effects and can be combined with other treatments.

Heat and cold therapy are simple interventions that many seniors find effective. Cold packs reduce acute inflammation and sharp pain, making them particularly useful in the first few days after an injury. Heat therapy increases blood flow and relaxes muscles, making it more suitable for chronic pain and stiffness. A typical application involves 15-20 minutes at a time, several times daily.
Manual therapy techniques performed by physical therapists or chiropractors can include massage, mobilization, and manipulation of the shoulder joint and surrounding tissues. These techniques aim to reduce muscle tension, improve joint mobility, and decrease pain. Research supports the use of manual therapy, particularly when combined with exercise programs.
Transcutaneous electrical nerve stimulation (TENS) units deliver mild electrical pulses through electrodes placed on the skin. The theory suggests these pulses interrupt pain signals and promote the release of endorphins. While some seniors report pain relief with TENS units, the scientific evidence supporting their long-term effectiveness is mixed.
Acupuncture, an ancient Chinese technique involving insertion of thin needles into specific body points, has shown promise for shoulder pain in some studies. Several research trials demonstrate that acupuncture can reduce pain and improve function in people with rotator cuff problems and frozen shoulder. The number of sessions needed varies, but typical treatment involves 6-12 sessions over several weeks.
Other complementary approaches that may provide support include:
- Massage therapy to reduce muscle tension and improve circulation
