What HPV Is and How It Spreads
Human papillomavirus (HPV) is a virus that infects the skin and mucous membranes of the body. There are more than 200 types of HPV, but only about 40 types can infect the genital area. The Centers for Disease Control and Prevention (CDC) reports that HPV is the most common sexually transmitted infection in the United States, affecting roughly 45 million Americans ages 18 to 59.

HPV spreads through skin-to-skin contact during sexual activity, which includes vaginal, anal, and oral sex. The virus does not spread through blood or bodily fluids like some other infections. A person can contract HPV from a sexual partner even if that partner has no visible symptoms or is unaware they carry the virus. This means transmission can occur without either person knowing infection has happened.
Most people who contract HPV—about 9 out of 10 people—will clear the infection naturally within one to two years without any treatment. The body's immune system typically recognizes HPV as a foreign invader and eliminates it. However, some people's immune systems do not clear the virus, leading to a persistent infection. Persistent infections with certain high-risk HPV types can cause cell changes over time that may develop into cancer.
HPV types are divided into two main categories: low-risk and high-risk. Low-risk types (such as HPV-6 and HPV-11) typically cause genital warts but rarely lead to cancer. High-risk types (such as HPV-16 and HPV-18) can cause cellular changes that develop into cervical cancer, anal cancer, throat cancer, and other cancers. Approximately 13 high-risk HPV types are linked to cancer development.
Practical takeaway: Understanding that HPV is common, often asymptomatic, and frequently resolved by the body on its own helps reduce stigma and panic. Most people infected with HPV will never develop symptoms or health problems.
The Early Stages: When Infection Begins
When someone first contracts HPV, they typically have no symptoms at all. This is the most common scenario—the CDC estimates that most people with HPV infection experience no noticeable signs. The virus begins its work at the cellular level, infecting the basal cells (the deepest layer of skin). During this initial phase, which can last weeks to months, a person may feel completely normal and have no reason to suspect infection.

The initial infection phase is when the virus is establishing itself but before any visible changes occur. During this time, the infected person may still transmit the virus to sexual partners, even though they show no symptoms. This asymptomatic period is one reason why HPV spreads so widely—people cannot tell they are infected and unknowingly pass it to others.
In the very early stages, a routine medical examination would not reveal anything unusual. Pap smears in people with cervices may not yet show abnormal cells. It can take several months to years of persistent infection before cellular changes become detectable through standard screening tests. The timeline varies significantly from person to person and depends on factors including the specific HPV type, immune system strength, age, smoking status, and overall health.
For people with penises, there are no routine screening tests for HPV like there are for people with cervices. This means early infection often goes completely unnoticed unless symptoms eventually develop. Similarly, throat and anal HPV infections have no standard screening tests for the general population, though high-risk individuals may have access to screening options.
During this early, asymptomatic stage, medical attention is typically not sought because the person has no reason to believe anything is wrong. The infection may be discovered by chance during routine sexual health screening or only becomes apparent if symptoms eventually develop or if screening tests show abnormal results.
Practical takeaway: The early stages of HPV infection produce no symptoms in most people. Regular screening (for those with cervices) and open communication about sexual health with partners are important because infection cannot be detected by symptoms alone.
Symptomatic Development: When Genital Warts Appear
If someone develops symptoms from HPV, genital warts are the most common manifestation. These warts typically appear between three weeks and eight months after infection, though the timing can vary widely—some people develop warts years after initial infection, and others never develop them at all. The CDC notes that genital warts affect approximately 1 percent of sexually active adults in the United States at any given time.

Genital warts appear as small, flesh-colored or slightly raised bumps in the genital area. They may be single lesions or appear in clusters. In people with vulvas, warts commonly develop on the labia, the opening of the vagina, or around the anus. In people with penises, warts typically appear on the shaft, head, or base of the penis, or around the anus. Warts can also develop inside the mouth or throat, though this is less common.
The appearance and progression of genital warts varies considerably. Some warts remain small and stable, while others grow larger or spread to additional areas. Some people experience rapid wart growth, while others see warts appear and disappear over months. This unpredictability can be emotionally difficult, as it creates uncertainty about the infection's course.
Symptoms accompanying genital warts may include itching, discomfort, or minor bleeding, particularly after sexual intercourse or during menstruation. Some people experience pain or burning sensations. However, many people with genital warts report no discomfort at all and only notice them visually or when a partner points them out. The presence of warts does not necessarily indicate a high-risk HPV type or increased cancer risk—low-risk HPV types cause the vast majority of genital warts.
It is important to note that not everyone infected with low-risk HPV types will develop visible warts. Many people clear the infection without ever showing any external signs. The development of warts depends on immune system response, the specific HPV type, and individual biological factors.
Practical takeaway: Genital warts are the most visible HPV symptom and typically appear months after infection. Their presence indicates low-risk HPV infection in most cases, not high-risk types associated with cancer.
Persistent Infection and Cell Changes Over Time
The critical difference between people who clear HPV naturally and those who develop serious health problems is whether the infection persists. Persistent HPV infection occurs when the body fails to eliminate the virus after initial infection. This happens in approximately 10 percent of people infected with HPV, particularly those infected with high-risk types. The reasons for persistent infection involve immune system variations, viral factors, and individual biological differences that researchers are still studying.

During persistent infection with high-risk HPV types, the virus integrates into the DNA of infected cells and causes gradual cellular changes. These changes do not happen overnight. Research shows that the process from persistent high-risk HPV infection to precancerous cell changes typically takes three to five years. The development from precancerous changes to actual cancer can take another five to ten years, though this timeline varies significantly.
In people with cervices, these cellular changes are detected through Pap smears or HPV tests. A Pap smear identifies abnormal cells; an HPV test identifies the presence of high-risk HPV types. When abnormal cells are found, they are classified using a system called the Bethesda System: ASC-US (atypical squamous cells of undetermined significance), LSIL (low-grade squamous intraepithelial lesion), HSIL (high-grade squamous intraepithelial lesion), or cancer. Not all abnormalities progress—many LSIL findings resolve naturally without treatment.
The progression from persistent HPV infection to cancer involves multiple steps. First, viral integration occurs. Second, infected cells begin showing abnormal growth patterns. Third, these abnormal cells accumulate additional genetic changes. Finally, after potentially many years, cells become truly malignant. This multi-step process means that persistent high-risk HPV infection does not automatically lead to cancer—most persistent infections, even with high-risk types, do not progress to cancer.
Factors that influence whether persistent infection progresses include immune system strength, smoking (which significantly increases risk), sexual behaviors, age at infection, and number of simultaneous HPV infections. People with weakened immune systems, such as
